Before volunteer service can begin, you must fill out the Volunteer Application, Availability Form, Disclosure Form, and State Patrol Disclosure Forms found at the NSD’s website. These 4 forms are considered up-to-date for 3 years. If you are not up to date, please complete all 4 forms and return them to the Timbercrest main office. The forms can be found on the NSD Volunteer Forms web page.
